The transitional Doctor of Physical Therapy (DPT) requires students to have a currently valid license in order to enroll and does not prepare students for licensure or certification.

Alabama	Alabama Board of Physical Therapy	The DPT program MEETS educational requirements to pursue professional licensure in Alabama.	All applicants must successfully pass a qualifying examination. Additional general requirements may apply including practice experience, state-specific coursework and exams, applications, fees, background checks, etc. US-educated students: Each applicant for licensure as a physical therapist must have completed a CAPTE approved program of physical therapy education appropriate for preparation as a physical therapist and successfully passed the National Physical Therapy Exam (NPTE). Non-United States educated applicants: All foreign educated therapists applying for a PT license in Alabama must complete a credentials review through FCCPT. A copy of the evaluation from FCCPT plus an English translated copy of the transcript must be sent directly to the board from this credentialing agency. Foreign educated physical therapist applicants must submit their education credentials directly to: Foreign Credentialing Commission on Physical Therapy, 124 West St., South; Third Floor, Alexandria, VA 22314, Telephone (800) 881-1430, Fax (703) 684-8715, www.fccpt.org. The Federation of State Board of Physical Therapy (FSBPT) provides an online tool to assist the foreign educated applicant to assess their status before they start the application process. The board shall make the sole judgment in determining whether educational credentials are equivalent and acceptable. All applicants: Complete a 2-hour Alabama Jurisprudence continuing education course within 30 days of licensure.
Alaska	State Physical Therapy and Occupational Therapy Board	The DPT program MEETS educational requirements to pursue professional licensure in Alaska.	All applicants must successfully pass a qualifying examination. Additional general requirements may apply including practice experience, state-specific coursework and exams, applications, fees, background checks, etc. Professional reference form completed by the head of the physical therapy school, instructor, physician, supervising physical therapist or supervisor and submitted directly to the Division from your professional reference. Verification of one of the following and submitted directly to the Division from the source: at least 60 hours of physical therapy employment within the 24 months immediately preceding the date the application is received by the department; passage of the national examination within the 24 months immediately preceding the date the application; 150 hour internship approved by the Board (contact the Division for further instructions regarding the internship); Jurisprudence questionnaire.
Arizona	Arizona State Board of Physical Therapy	The DPT program MEETS educational requirements to pursue professional licensure in Arizona.	All applicants must successfully pass a qualifying examination. Additional general requirements may apply including practice experience, state-specific coursework and exams, applications, fees, background checks, etc. An applicant for a license as a physical therapist who has been educated in the United States shall: 1. Be of good moral character. 2. Complete the application process. 3. Be a graduate of a professional physical therapy education program that is accredited by a national accreditation agency approved by the board. 4. Have successfully passed the national examination approved by the board. 5. Have successfully passed a jurisprudence examination that tests the applicant’s knowledge of board statutes and rules. 6. Obtain a valid fingerprint clearance card.
Arkansas	Arkansas State Board of Physical Therapy	The DPT program MEETS educational requirements to pursue professional licensure in Arkansas.	All applicants must successfully pass a qualifying examination. Additional general requirements may apply including practice experience, state-specific coursework and exams, applications, fees, background checks, etc. Passing score is 70% on Arkansas jurisprudence exam. Passing score on a national physical therapy examination approved by the Arkansas State Board of Physical Therapy. Official verification of all current and/or all previously issued licenses to practice physical therapy.
California	Physical Therapy Board of California	The DPT program MEETS educational requirements to pursue professional licensure in California.	All applicants must successfully pass a qualifying examination. Additional general requirements may apply including practice experience, state-specific coursework and exams, applications, fees, background checks, etc. Every applicant for a license under this chapter shall, at the time of application, be a person over 18 years of age, not addicted to alcohol or any controlled substance, have successfully completed the education and training required by Section 2650, and not have committed acts or crimes constituting grounds for denial of licensure under Section 480. Except as otherwise provided in this chapter, no person shall receive a license under this chapter without first successfully passing the following examinations, where success is determined based on the examination passing standard set by the board: An examination under the direction of the board to demonstrate the applicant’s knowledge of the laws and regulations related to the practice of physical therapy in California (CAL-Law). The physical therapy examination for the applicant’s licensure category (NPTE). The applicant must also submit an application, fee, passport photo, fingerprint, and certificate of completion (P1E). An applicant may be issued a license without a written examination if he or she meets all of the following: He or she is at the time of application licensed as a physical therapist or physical therapist assistant in a state, district, or territory of the United States having, in the opinion of the board, requirements for licensing equal to or higher than those in California, and he or she has passed, to the satisfaction of the board, an examination for licensing that is, in the opinion of the board, comparable to the examination used in this state. He or she is a graduate of a physical therapist or physical therapist assistant education program approved by the board, or has met the requirements of Section 2653. He or she files an application with the board and meets the requirements prescribed by Sections 2635 and 2650. An applicant for licensure under subdivision (a), whose application is based on a certificate issued by a physical therapy licensing authority of another state may be required to file a statement of past work activity.
Colorado	Colorado State Physical Therapy Board	The DPT program MEETS educational requirements to pursue professional licensure in Colorado.	All applicants must successfully pass a qualifying examination. Additional general requirements may apply including practice experience, state-specific coursework and exams, applications, fees, background checks, etc. Applicants must be graduates of a CAPTE accredited academic program and must successfully pass the national licensing examination. Criminal background check is required.
Connecticut	Connecticut State Department of Public Health	The DPT program MEETS educational requirements to pursue professional licensure in Connecticut.	All applicants must successfully pass a qualifying examination. Additional general requirements may apply including practice experience, state-specific coursework and exams, applications, fees, background checks, etc. Successful completion of a program of study approved by the American Physical Therapy Association (APTA); successful completion of the National Physical Therapy Examination (NPTE) administered by the Federation of State Boards of Physical Therapy; criminal background check. Department of Public Health may “deny licensure to an individual who has committed or been found guilty of committing a felony or an act which does not conform to the accepted standards of the profession.”
Delaware	Delaware Division of Professional Regulation	The DPT program MEETS educational requirements to pursue professional licensure in Delaware. Florida	Florida Board of Physical Therapy	The DPT program MEETS educational requirements to pursue professional licensure in Florida.	All applicants must successfully pass a qualifying examination. Additional general requirements may apply including practice experience, state-specific coursework and exams, applications, fees, background checks, etc. Applicants must have received a degree as a physical therapist from an institution that has been approved for the training of physical therapists by the Commission on Accreditation for Physical Therapy Education (CAPTE) and pass the National Physical Therapy Examination (NPTE).
Georgia	Georgia State Board of Physical Therapy	The DPT program MEETS educational requirements to pursue professional licensure in Georgia.	All applicants must successfully pass a qualifying examination. Additional general requirements may apply including practice experience, state-specific coursework and exams, applications, fees, background checks, etc. Applicants must be graduates of a Commission on Accreditation in Physical Therapy Education (CAPTE) accredited school; pass the National Physical Therapy Exam (NPTE); and pass the Georgia Jurisprudence (Georgia Law) exam.
Hawaii	Board of Physical Therapy	The DPT program MEETS educational requirements to pursue professional licensure in Hawaii.	All applicants must successfully pass a qualifying examination. Idaho	Physical Therapy Licensure Board	The DPT program MEETS educational requirements to pursue professional licensure in Idaho.	All applicants must successfully pass a qualifying examination. Additional general requirements may apply including practice experience, state-specific coursework and exams, applications, fees, background checks, etc. All applicants must provide documentation of their education and proof of graduation from a nationally accredited school of physical therapy approved by the board. Applicants are also required to disclose any criminal convictions other than minor traffic infractions.

Applicants must be of good moral character, graduate from an APTA-accredited program, and pass the NPTE
Michigan	Michigan Department of Licensing and Regulatory Affairs	The DPT program MEETS educational requirements to pursue professional licensure in Michigan.	All applicants must successfully pass a qualifying examination. Additional general requirements may apply including practice experience, state-specific coursework and exams, applications, fees, background checks, etc. Applicants must graduate from a CAPTE accredited PT program and submit official transcripts, pass the NPTE exam, score 75% or higher on a jurisprudence exam, and complete a LIVE Scan Fingerprint background check, complete the Michigan application. In addition to the above, applicants whose education is not CAPTE-accredited whose educational program was not located in Australia, a province of Canada that is not Quebec, Ireland, New Zealand, the United Kingdom, or the United States must obtain an 89 on the TOEFL-IBT, including at least 22 on the reading section, 21 on the listening section, 24 on the speaking section, and 22 on the writing section. PT applicants whose education is not CAPTE-accredited must have their education evaluated using the Foreign Credentialing Commission on Physical Therapy (FCCPT) course work evaluation tool.
Minnesota	Minnesota Board of Physical Therapy	The DPT program MEETS educational requirements to pursue professional licensure in Minnesota.	All applicants must successfully pass a qualifying examination. Additional general requirements may apply including practice experience, state-specific coursework and exams, applications, fees, background checks, etc. Applicants must submit an online application, followed by: Evidence satisfactory to the board that the applicant has met the educational requirements of section 148.721 or 148.722 as demonstrated by a certified copy of a transcript. (The transcript must be sent directly to the MN Board of PT office from the school) Two recommendation forms submitted by two physical therapists registered or licensed to practice physical therapy in the United States or Canada. (Choose two physical therapist references from those listed on page 3 of your application to complete the recommendation forms. They must be mailed to the Board office by the individual filling out the form) A recent full faced photograph of the applicant attached to the application with the affidavit on the form completed and notarized. (2×3 photograph printed on photo paper affixed) A record of the applicant’s high school, college and board-approved physical therapy school education listing the names, locations, dates of attendance and diplomas, degrees or certificates awarded. (All time must be accounted for on the application from high school to the date of application. During continuous years of education, period of three months or less (summer break) need not be accounted for) A record of postgraduate work and military service. (Notarized copy of military discharge papers, if applicable) If applicable, a listing of the United States jurisdictions, and countries in which the applicant is currently licensed or registered or has been in the past. A record of disciplinary action taken on past complaints, refusal of licensure or registration, or denial of examination eligibility by another state board or physical therapy society against the applicant. A record of the applicant’s personal use or administration of any controlled substances and treatment for alcohol or drug abuse.
Mississippi	Mississippi State Board of Physical Therapy	The DPT program MEETS educational requirements to pursue professional licensure in Mississippi. If the applicant has not yet taken the NPTE, they must also submit agreement to supervise form, certificate of professional education, and an accommodation request form (as needed).
Montana	Montana Board of Physical Therapy Advisors	The DPT program MEETS educational requirements to pursue professional licensure in Montana. North Dakota	North Dakota Board of Physical Therapy	The DPT program MEETS educational requirements to pursue professional licensure in North Dakota.	All applicants must successfully pass a qualifying examination. Additional general requirements may apply including practice experience, state-specific coursework and exams, applications, fees, background checks, etc. Complete the online license application as a Physical Therapist or submit a completed application to the ND Board of PT office; Payment can be made online using a credit card or enclose a check for $241.25 payable to the ND Board of PT, $200 for the non-refundable application fee and $41.25 for the criminal background check Juris Prudence: This is an open book exam which can be completed by using up the Rules & Regulations and ND PT Practice Act. Applications for the NPTE must be done online through the FSBPT. Your school will need to notify FSBPT that you have either graduated or are on track to graduate. North Dakota participates in the alternate approval pathway. FSBPT will make you eligible to sit for the exam, not the ND board; An official transcript indicating completion of an accredited physical therapist program must be sent directly from the school of graduation prior to being issued a license. North Dakota does accept transcripts by electronic means if sent directly from the school of graduation.
Ohio	Occupational Therapy, Physical Therapy, and Athletic Trainers Board	The DPT program MEETS educational requirements to pursue professional licensure in Ohio.	All applicants must successfully pass a qualifying examination. Additional general requirements may apply including practice experience, state-specific coursework and exams, applications, fees, background checks, etc Applicants must graduate from an accredited PT program, pass the NPTE exam and the Ohio Law Exam or Jurisprudence Assessment Module. Must submit BCI/FBI criminal records check.
Oklahoma	Oklahoma Medical Board	The DPT program MEETS educational requirements to pursue professional licensure in Oklahoma.	All applicants must successfully pass a qualifying examination. Oregon	Oregon Board of Physical Therapy	The DPT program MEETS educational requirements to pursue professional licensure in Oregon.	All applicants must successfully pass a qualifying examination. Additional general requirements may apply including practice experience, state-specific coursework and exams, applications, fees, background checks, etc. A completed application includes all of the following: Application form and fee Oregon jurisprudence exam (OR-JAM) with passing score of 80 or above; Certificate for completion of the Pain Management Module (PT’s only); Criminal background check – Fieldprint; Proof of graduation from a CAPTE accredited program or, for foreign educated therapist, the appropriate Course Work Tool showing substantially equivalent education, and proof of English equivalency; Passing NPTE score; and(7) Verification of all health care licenses ever held, if any.
Pennsylvania	State Board of Physical Therapy	The DPT program MEETS educational requirements to pursue professional licensure in Pennsylvania.	All applicants must successfully pass a qualifying examination. Additional general requirements may apply including practice experience, state-specific coursework and exams, applications, fees, background checks, etc. Applicants must graduate from an accredited program and submit verification of education form, must pass NPTE exam and have score transferred through FSBPT. Other items include application and fee, child abuse continuing education course, criminal history check, curriculum vitae, National Practitioner Databank Report, and Letter of Good Standing if previously licensed in another state/territory.
Rhode Island	State of Rhode Island Department of Health	The DPT program MEETS educational requirements to pursue professional licensure in Rhode Island.	All applicants must successfully pass a qualifying examination. Additional general requirements may apply including practice experience, state-specific coursework and exams, applications, fees, background checks, etc. Applicants must graduate from an accredited program and submit official transcripts, must pass NPTE exam and have score transferred through FSBPT. Other items include application and fee, full bureau of criminal identification report from RI attorney general’s office. If transferring license, need license verification from current state.
South Carolina	South Carolina Board of Physical Therapy	The DPT program MEETS educational requirements to pursue professional licensure in South Carolina.	All applicants must successfully pass a qualifying examination. Additional general requirements may apply including practice experience, state-specific coursework and exams, applications, fees, background checks, etc. To be licensed by the board as a Physical Therapist in South Carolina you must: be a graduate of a physical therapy educational program approved by the board or provide satisfactory evidence that your education is equivalent to the requirements of physical therapists educated in United States educational programs as determined by the board. If the board determines that an applicant’s education is not equivalent, it may require completion of additional course work before proceeding with the application process; pass an examination administered or approved by the board; speak the English language as a native language or demonstrate an effective proficiency of the English language in the manner prescribed by and to the satisfaction of the board; and submit evidence satisfactory to the board on a form approved by the board of not less than one thousand clinical practice hours under the on-site supervision of a licensed physical therapist in this State or in a state with licensure requirements equal to or more stringent than this State.
South Dakota	South Dakota Board of Medical and Osteopathic Examiners	The DPT program MEETS educational requirements to pursue professional licensure in South Dakota.	All applicants must successfully pass a qualifying examination. An applicant for a license as a physical therapist who has been educated outside the United States and has graduated from a professional physical therapy education program not accredited by a national accreditation agency approved by the Director shall provide satisfactory evidence that the applicant’s education is substantially equivalent to the requirements of physical therapists educated in an accredited education program as determined by the Director. For the purpose of this section, “substantially equivalent” means that an applicant for licensure educated outside the United States shall have: graduated from a physical therapist education program that prepares the applicant to engage in the practice of physical therapy without restriction; provided written proof that the applicant’s school of physical therapy education is recognized by its own ministry or department of education; undergone a credentials evaluation as directed by the Director that demonstrates that the candidate has met uniform criteria for educational requirements as adopted by rule; and completed any additional education as required by the Director pass an approved English proficiency examination if the applicant’s native language is not English; and pass an examination approved by the Director.

Please be advised that effective June 7, 2010 all sections of the TOEFL must have passing scores on one test, and multiple tests cannot be combined. You may be exempt from this requirement if you have been awarded a bachelor’s degree or higher from the United States, United Kingdom, Ireland, Australia, New Zealand, or Canada (except Quebec). If this exemption applies, your school must submit an official transcript showing that the degree was awarded and the date of conferral
