SAS Student Registration Form (required SAS intake form)
REQUIRED FOR ALL NEW ACCOMMODATION REQUESTS- Students requesting approval for accommodations at Marymount must submit a completed "SAS Registration Form" to Student Access Services (SAS) directly, along with the "SAS Information Release Form" (below) and their disability documentation. The requesting student is responsible for logging into Starfish to schedule a phone, video, or in-office SAS intake meeting (locate staff calendars under "My Success Networks" in Starfish).
- This form is reviewed before or during the student's SAS intake meeting. It is used in conjunction with the student's disability documentation (and any other required SAS forms) to identify the most appropriate accommodations for the student at MU.
SAS Student Information Release Form (required SAS intake form)
REQUIRED FOR ALL NEW ACCOMMODATION REQUESTS- Students must submit this completed "SAS Information Release Form" with their "SAS Registration Form" and disability documentation, to the SAS office directly. The requesting student is responsible for logging into Starfish to schedule a phone, video, or in-office SAS intake meeting (locate staff calendars under "My Success Networks" in Starfish).
- This form indicates who (if anyone) the SAS office is allowed to speak with or respond to (i.e., family members, advocates/support people, medical providers, etc.) should they have questions about student's disability status or SAS-approved accommodations. The SAS office cannot release information or discuss the student's confidential SAS records with anyone who the student has not listed on this form.
In addition to the two forms linked above, students who wish to request housing or dining accommodations (due to a diagnosed disability) must also submit a "Housing-Dining Accommodations Request Form" and supporting medical documentation to SAS, directly.
- The requesting student is responsible for specifying their housing/dining accommodation request on the forms, and for logging into Starfish to schedule a phone, video, or in-office meeting with the Director of SAS. Additional documentation may be required from the student, depending on the requested housing/dining accommodation.
Students who are approved for the use of alternative textbook formats (i.e., digital, audio) may request them through SAS, after they have purchased the textbook hardcopy on their own. Students should submit an individual form for each textbook and scan an attachment to Access. It may take up to four weeks to receive the alternative format, so students should submit the requests as early as possible. Additional instructions are described on the form.
Students who require a sign language interpreter for an MU class or MU-sponsored event must submit this completed request form to SAS, at least 2-3 weeks prior to the date that an interpreter will be needed: Access, or 703-284-1538.
Students who require personal care attendant services who wish to have the same independent experience as all other college students are encouraged to hire an impartial personal care attendant (PCA), who is not a family member or close friend. Any student requiring the use of a PCA must submit this form in order to register the PCA as a member of the Marymount community.
Please complete and submit the form to Access (or fax to 703-284-6485), if you or one of your invited guests will need disability accommodations for any of MU's graduation events (e.g., wheelchair accessible seating, seating near the entrance door or restroom, access to sign language interpreters).
- This form can be obtained online here, or from the front desk of the Academic Affairs/Provost's Office on the 1st floor of Rowley. SAS does not provide tickets for graduation events. Accommodated seating cannot be guaranteed. This form should be submitted to the SAS e-mail ( Access) or fax number (703-284-6485), at least 3 weeks before the graduation event.
SAS Faculty Forms
Faculty members need to submit this form to the Center for Teaching and Learning with each exam for students with approved testing accommodations using the CTL for accommodated test proctoring services.
Facutly members sign after meeting with students who have approved accommodations each semester.
Professors, staff and invited guests who require a sign language interpreter or captioning services must submit this completed request form to SAS, at least 2-3 weeks prior to the date that services will be needed: Access, or 703-284-1538.
