Academic Internship Agreement Form - Use this document to provide details of your internship, secure signatures, and submit to register your internship. This document must be completed and turned into your AIM before you can begin working at your internship site.
Student Internship Site Evaluation Form - Use this form to evaluate your internship experience and work site. This document must be turned into your AIM before your internship experience is complete.
Employer Evaluation Form - Supply your internship site supervisor with this document prior to concluding your internship. Your internship site supervisor should complete the form and return it to your AIM before your internship ends. Be sure to utilize the appropriate employer evaluation forms for your academic program.
